Divine Mercy Homes is an assisted living facility licensed to serve people 55 years and older with disabilities and chronic medical conditions such as stroke, diabetes, MS, wound care, etc. We also care for people with mental and behavioral health issues. We also offer medication management, arrange transportation and schedule medical appointments for our residents in addition to providing them with ADLs. We understand that the need for these groups of people is on the rise and can be overwhelming for family members. Therefore, there is a need for a place like ours where the residents are treated and cared for by well-trained and dedicated staff.
We at Divine Mercy Homes pride ourselves in providing person-centered care to all our residents, irrespective of their medical or behavioral health conditions.
We are constantly researching and collaborating with the Minnesota Department of Health, Minnesota Department of Human Services, and the Care Providers of Minnesota to update ourselves with the latest and up-to-date information on in-home care and assisted living facility services.
